چکیده

Background and study aims: The aim of this study was to evaluate the efficacy and safety of the laparoscopic common bile duct exploration performed within 24 hours from the urgent hospitalisation of patients with calculous cholecystitis associated to choledocholithiasis either proven or suspected. Patients and methods: From November 2010 to May 2012, 126 patients hospitalised at the Regional Hospital of Orleans (France) were retrospectively reviewed. All patients were preoperatively assessed only by means of routine blood tests and abdominal ultrasounds. Results: In our study were collected: 67 choledochotomies and 59 transcystic approaches; the laparoscopic procedure was successfully achieved in 97.6% of the cases. Statistical significant differences were found about common bile duct dilatation (P < 0.05) and in-hospital stay (P < 0.001). Conclusion: We suggest that the laparoscopic-first approach for choledocholithiasis, realised in specialised centres by skilled surgeons in hepatobiliary diseases, represents a feasible, safe and cost-effective management also in patients urgently admitted and treated for acute calculous cholecystitis.

Duplicated common bile duct: a recipe for biliary injury. Report of a new variant

Duplicated common bile duct, often associated with conditions like lithiasis, biliary cysts and pancreatobiliary maljunction, could result in highly morbid and potentially fatal biliary injuries. Precise preoperative diagnosis and classification still remain a challenge. A female patient undergoing emergency laparoscopic cholecystectomy for acute calculous cholecystitis sustained iatrogenic bil...

One Thousand and Six Consecutive Laparoscopic Intraoperative Cholangiograms

Intraoperative cholangiography was successfully performed in 1,000 out of 1,006 attempts in 1019 consecutive cholecystectomies. There were 783 chronic, 95 acute, 61 fibrotic, 27 gangrenous and 40 cases of hydrops of the gallbladder in those laparoscopic cholecystectomies performed. unsuspected common duct stones were identified in 5% of the patients. There were no injuries resulting from intrao...

Elective Laparoscopic Cholangiography in Lithiasic Pathology. Intraoperative Selection Criteria.

INTRODUCTION The introduction of laparoscopic techniques has caused the dispute between supporters of routine or elective intraoperative cholangiography to continue, but at present most authors recommend its elective practice on the basis of well-established preoperative and / or intraoperative indications, thus avoiding to carry out a large number of unnecessary procedures, with their inherent...

Acute Calculous Cholecystitis: What is new in diagnosis and therapy?

The management of patients with acute calculous cholecystitis has changed during recent years. The etiology of acute cholecystitis is still not fully understood. Infection of bile is relatively unimportant since bile and gallbladder wall cultures are sterile in many patients with acute cholecystitis. Ultrasonography is first choice for diagnosis of acute cholecystitis and cholescintigraphy is s...
